چکیده
Course Multiple aspirations of the right shoulder and both knee joints were attempted during the first 4 days; little fluid was obtained in spite of the continued presence of clinical signs of infection. Throughout the first week ofhospitalization, the patient remained toxic with a daily temperature of 103-104°F. On the seventh day arthrotomy of each affected joint was performed and drains were inserted. Cultures of the joint fluid were sterile. The clinical course was stormy with persistent fever and swollen, painful joints for 5 weeks.

Rapid diagnosis of septic arthritis by coagglutination.
Pneumococcal capsular antigens were detected and serotyped by coagglutination in joint fluids and serum of a patient with septic arthritis within 1 h of obtaining the specimens. Pneumococcal antigens continued to be detected by coagglutination for 3 days, whereas cultures and Gram stains were negative after 1 day of antibiotic therapy.
متن کاملThe association between antibody levels before and after 7-valent pneumococcal conjugate vaccine immunization and subsequent pneumococcal infection in chronic arthritis patients
INTRODUCTION The aim of present study is to inverstigate the association between antibody levels after vaccination with 7-valent pneumococcal conjugate vaccine (PCV7) and subsequent serious pneumococcal infections in rheumatoid arthritis (RA) and spondylarthropathy (SpA) patients. METHODS A cohort of 497 patients (RA=248 and SpA=249) received a single dose of PCV7. At vaccination, patients we...
متن کاملRates of, and risk factors for, septic arthritis in patients with invasive pneumococcal disease: prospective cohort study

A 50 year old woman presented with pneumococcal septicaemia, septic arthritis, and a lobar pneumonia and was subsequently diagnosed as having systemic lupus erythematosus. The blood film and splenic 99mTc sulphur colloid uptake were normal, although selective functional hyposplenism was shown by the impaired clearance of immunoglobulin coated erythrocytes. Systemic lupus erythematosus presentin...

A patient with Felty's syndrome who developed bilateral knee septic arthritis and septicaemia due to Streptococcus pneumoniae is described. She had had a previous splenectomy for symptomatic thrombocytopenia, having received pneumococcal vaccine before the operation. Measurement of antibody to the 23 vaccine serotypes showed protective concentrations before infection to just two. The infecting ...
متن کاملVaccination in Patients with Rheumatoid Arthritis Receiving Immunotherapies
Patients with rheumatoid arthritis (RA) are at higher risk to suffer from morbidity due to vaccine-preventable diseases and, thus, display an important target population to receive vaccines for protection from infectious complications. There have been only a few studies focusing on the administration of vaccines in RA patients with immunotherapy. Overall, antibody response rates against influen...
